The longevity industry sells optimization to people who haven't built a baseline. A field guide to the unglamorous fundamentals — and to evaluating the clinics selling everything else.

Longevity medicine has a marketing problem that works in its favor: the products are exciting and the fundamentals are not. Peptide stacks, NAD drips, and hormone optimization make compelling brochures. Strength training, sleep, and a protein target make none. So the industry's center of gravity sits exactly where the evidence is thinnest and the margins are thickest — and a person who wants to "do longevity" mostly gets offered the expensive periphery first.

This piece is the sequencing argument we wish every clinic made unprompted: what to build first, what the clinic layer is actually for, and how to interview anyone who wants to optimize you.

The boring portfolio

The interventions with the deepest evidence for healthspan are embarrassingly available. They're also where the compounding lives — every year they're in place multiplies the value of every year after.

  • Strength is the retirement account. Muscle mass and strength track with essentially every outcome people care about late in life — mobility, metabolic health, surviving the falls and illnesses that end independence. Two to three resistance sessions a week, progressively loaded, is the single most defensible "protocol" in this entire field.
  • Cardiorespiratory fitness is the second account. However you measure it, the relationship between aerobic capacity and long-term health is among the most consistent findings in exercise science. Mostly-easy volume, some genuinely hard intervals, week after week.
  • Sleep is when the maintenance runs. Chronic short sleep degrades metabolic, cognitive, and immune function on timelines short enough to feel. It's also the variable that makes every other variable easier or harder.
  • Protein and the absence of metabolic excess do more than any supplement stack. Adequate protein to support the strength work; a diet that keeps energy balance and blood markers in range. The specific dietary religion matters far less than its adherents believe.

None of this requires a clinic. All of it requires consistency, which is the actual scarce resource in longevity — and the honest explanation for why purchased protocols are so seductive. Buying feels like progress.

What the clinic layer is actually for

Used well, longevity medicine is a measurement-and-medical layer on top of that base, not a replacement for it. Baseline labs establish where you actually stand — lipids, glucose regulation, hormones, the markers your physician can act on — and repeated over years, they turn vague intentions into a managed trajectory. Genuine deficiencies and diagnosed conditions get treated by the standard of care: thyroid disease, clinically low testosterone with symptoms, menopause — real medicine, well inside the evidence.

Where it gets murky is the gap between treating a deficiency and "optimizing" a normal value. Hormone therapy for a diagnosed deficit is medicine; chasing the top of the reference range in a healthy person is a bet, with real risk, made on thin evidence. Peptides span everything from FDA-approved drugs to gray-market compounds with no human outcome data sold on forum enthusiasm. NAD infusions rest on interesting cellular biology and essentially no demonstrated clinical outcomes in healthy people — what you're buying, mostly, is plausibility plus an IV pole.

The question is never "does the mechanism sound good?" Every mechanism sounds good. The question is "what happens to people like me who take this for years — and does anyone actually know?"

Interviewing a longevity clinic

  • "What would you have me do that costs nothing?" — The honest ones lead with the boring portfolio above. A clinic whose plan for you is entirely purchasable is describing its business model, not your biology.
  • "What's the evidence tier for this — approved indication, off-label with trials, or mechanism-and-hope?" — A good physician keeps these categories distinct, tells you which one you're in, and prescribes accordingly. Blurring them is the field's signature sin.
  • "What are we measuring, and what result would make you stop?" — Optimization without a stopping rule is a subscription. You want defined markers, defined recheck intervals, and a stated willingness to discontinue what isn't working.
  • "Who's medically accountable here?" — Same as a med spa: a licensed physician whose name is on your chart, reachable when something goes wrong, with the license lookup to match.

The sequence, restated

Build the base — it's free, it's proven, and it compounds. Add measurement so decisions stop being vibes. Treat what's actually diagnosed, with a physician who distinguishes medicine from optimization. And hold the exciting periphery to the standard it keeps dodging: not whether it might work, but whether anyone can show it does.

The strongest evidence points to unglamorous fundamentals: regular physical activity (especially strength training), adequate sleep, a whole-food diet, not smoking, moderate alcohol, and maintained social connection. These outperform most supplements and protocols and are the foundation any longevity plan should start from.
Are peptides and hormone protocols necessary for longevity?
For most healthy people, not before the fundamentals are in place. Hormone therapy has legitimate, evidence-based uses (menopausal symptoms, diagnosed hypogonadism) but should be guided by testing and a qualified physician, not used speculatively for 'optimization.' Discuss risks and monitoring with an endocrinologist.
Should I get hormone therapy to feel younger?
Hormone therapy is a medical treatment with real benefits and real risks, appropriate for specific diagnoses. Testosterone therapy for men and estrogen therapy for menopausal women are guided by symptoms plus lab confirmation and ongoing monitoring. Using hormones without a clear indication can carry risk; the decision belongs with an endocrinologist.
Do longevity supplements work?
Most lack strong human evidence for extending lifespan, and supplements aren't tightly regulated for efficacy. Some have reasonable data for specific deficiencies (e.g., vitamin D when low). Before spending on a supplement stack, invest in sleep, exercise, and diet — and tell your doctor what you take, since interactions are real.
